Acceptance of Terms
By accessing, purchasing from, or using services provided by Crossroads Medical Supply (“Company,” “we,” “us,” or “our”), you agree to be bound by these Terms of Service (“Terms”). If you do not agree, you must not use our services.
Services Provided
We provide durable medical equipment (“DME”), supplies, and related services, including but not limited to:

All services are subject to availability and applicable federal and Texas laws.
Eligibility
You must be at least 18 years old or have a legally authorized representative.
By using our services, you represent that:

You are authorized to use any insurance submitted
Physician Orders & Medical Necessity
Certain items require a valid physician order.

We may verify medical necessity and documentation

All orders must comply with Medicare, Medicaid, and commercial payer requirements

We reserve the right to refuse service if documentation is incomplete or non-compliant
Insurance & Payment Responsibilities
We may bill insurance as a courtesy.
However, you are ultimately responsible for all charges, including:

Deductibles

Copayments
-
Coinsurance
-
Non-covered items
-
Denied claims
If insurance denies payment for any reason, you agree to pay the remaining balance. We may require payment in advance for certain items or services
Rentals & Equipment Use
Rental equipment remains the property of Crossroads Medical Supply unless purchased.
You agree to:

Maintain equipment in good condition
Report damage or malfunction promptly You are responsible for:

Loss, theft, or damage beyond normal wear and tear
Failure to return equipment may result in additional charges and legal action.
Returns & Refunds
Due to hygiene and safety regulations:

Many medical items are non-returnable

Eligible returns require prior authorization

Items must be unused and in original condition
Restocking fees may apply. Custom or special-order items are non-refundable.
Delivery & Setup
Delivery times are estimates and not guaranteed.

We offer recurring resupply programs for CPAP and CGM supplies.

Frequency
Supplies are shipped based on:

Patient Responsibilities
You agree to notify us of:

Insurance Billing
We may bill insurance for each shipment. You remain responsible for all patient balances.
Confirmation
We may contact you prior to shipment. Failure to confirm may delay service.
Auto-Ship Authorization
By enrolling in auto-ship, you authorize us to:

Ship supplies on a recurring basis

Bill your insurance and/or payment method on file
Payment Authorization
You authorize charges for:

Recurring shipments

Outstanding balances
Cancellation
You may cancel or modify auto-ship with reasonable notice prior to shipment.
Failed Payments
We may delay or cancel shipments if payment fails.
Equipment Maintenance & Service
We may provide maintenance and service agreements.
You agree to:

Report issues promptly
Service does not cover damage due to misuse, neglect, or unauthorized modification.
Collections & Late Payment Policy
Payment Terms
are due upon receipt unless otherwise agreed.
Delinquent Accounts

Collections
Unpaid accounts may be referred to collections.
You agree to pay all lawful collection costs, including:

Collection agency fees

Returned Payments
Fees may apply for returned or failed payments.
